Perfect Arreat's Face is helmin Blizzard North'scRPGof 2000,Diablo 2. As a rule of thumb, barbarian helms don't get any better than the Arreat's Face slayer guard unique:

We don't want ethereal because then we have to Zod the ethereal or give it to an Act V merc. Arreat's Faces are usually Ber'd though some godly jewels qualify, too.
